This review will happen after your 14th birthday in year 9.

Other people who might attend the meeting are:

  • Your teachers
       
  • Your Connexions Personal Adviser  
      
  • Your key worker (if you have one)  
       
  • A social worker (if you have one).
What needs to be done

After the meeting a Transition Plan will be written. It will say:

  • What you want to do next    
       
  • How you will do it    
       
  • Who will help you. 

To keep it up to date the Transition Plan will be looked at every year.
